FDC Patient/Provider Agreement
The office policy agreement between you and the practice.
If a form will not open on your device, call the front desk on 406-727-1006 and a copy will be ready for you at the office instead.
One less thing on the day.
We accept and file most types of insurance and are providers for Delta Dental (Premier level), MetLife Insurance, and United Concordia Tricare Dental program. If you are not certain where your plan lands, ask before you book and the front desk will check it.
